Create Multiple Certificates from One Form Response

Smart Certificates allows you to generate more than one certificate from a single form submission. This is helpful when:

• A participant is eligible for multiple achievements.

• You want to generate certificates for different roles (e.g., participant + mentor).

• You offer multi-part courses or events and want to issue a certificate for each.

How It Works

When configuring certificate settings, you can enable and define multiple templates:

• Each template is treated as a separate certificate.

• You can assign different placeholders, certificate names, and output formats for each one.

• All certificates will be generated in a single submission event.

Steps to Enable

• Open the Smart Certificates add-on from your Google Form.

• In the Certificate Settings tab, click “+ Add Template” to configure a second certificate.

• For each template:

• Select a Slides template.
• Configure placeholder mapping.
• Set a certificate name and folder.
• Choose the output format (PDF or Image).

• Repeat this process for as many templates as needed.

• Save the settings and test by submitting your form.
